To support patients with their health and wellbeing through connecting them with local and national support. Social Prescribing gives time to patients to discuss the issues they are facing and work through how they can be supported.
What I can do
Referrals can be for:
- Accessing support for wellbeing/mood
- Financial worries
- Housing issues
- Volunteering/training
- Support for carers
- Loneliness and isolation
- Bereavement
- Signposting to local and national peer support groups
How to refer
If patients would like to be referred, care navigators can book appointments in, with a small description so that Becky is aware of what support may be needed.
Social Prescribing session example
Usually, Becky would talk with a patient for an hour on average, the first call with a patient can be longer. Discussions on the reason for the referral, what they want to achieve, and assess what is happening with the patient outside of this referral.
